Over 50.6 million Americans lack access to primary health care; in Illinois alone, 1,764,6001 are without insurance, and in 2009, 1 in 8 adults avoided doctor visits because of cost2. To ensure that the underserved get the health care they need, planning for Greater Elgin Family Care Center (GEFCC) began as a community-based grass-roots concern in 1996.

The primary purpose of GEFCC is to provide comprehensive medical care to individuals in the greater Elgin community, especially those who:

- Have No Source of Medical Insurance
- Are Medicaid or Medicare Recipients
- Are AllKids and FamilyCare Recipients

Opened in the summer of 2001, Greater Elgin Family Care Center is able to serve any individual or family needing health care and a medical home. Currently providing care at 28 different locations including schools, churches, and Elgin Community College. GEFCC offers:

Collaborations

Since its inception, Greater Elgin Family Care Center has collaborated extensively with other entities with the goal of increasing efficiency and minimizing duplication—all for the betterment of our patients. The following is a list of some of our current collaborations:

Aunt Martha's Youth Service Center: GEFCC provides its quality, affordable primary healthcare services to homeless persons from Elgin with financial support from Aunt Martha’s.

Boys & Girls Club of Elgin: GEFCC offers both primary care and dental care to Boys and Girls Club members and parents through their annual health fairs.

Centegra Health System: Centegra leases space from GEFCC in our McHenry Community Health Center, mirroring the hours that the Health Center is open. GEFCC Licensed Independent Practitioners have secure, private access to images and all health record information of McHenry Community Health Center patients who receive care, including ultrasounds, at Centegra Hospital McHenry.

Centro de Información: GEFCC provides health education for Centro clients at Centro’s offices twice monthly. Additionally, GEFCC offers both primary care and dental care to Centro clients through their annual health fairs.

Community Coordinated Child Care (4-C): Our McHenry Community Health Center professionals are actively working on a committee formed by 4-C to improve access to services available to those children of McHenry County in need.

Community Crisis Center: GEFCC provides primary care services to the residents of CCC in the shelter on a regularly-scheduled basis.

Ecker Center for Mental Health is where GEFCC refers Elgin-area adult patients who are in need of outpatient behavioral health services. Conversely, Ecker Center refers their clients to GEFCC for primary health care, including adult oral health care, as needed.

Elgin Community College: GEFCC volunteer dentists provide oral health care in conjunction with ECC’s Dental Assistance Program in the College’s Dental Assistance classroom and operatories.

Family Service Association of the Greater Elgin Area: In addition to GEFCC referring it behavioral health patients to FSA for ongoing counseling, the two organizations jointly provide behavioral health services to U-46 students in several of the District’s schools.

Kane County Family Case Management Cluster: GEFCC, as the current Cluster Chair, coordinates communication and integration amongst Kane County’s Family Case Management providers including Aunt Martha’s Youth Service Center and VNA of Fox Valley.

Laboratory Corporation of America: LabCorp leases space in our Seneca Health Center, providing a comprehensive Patient Service Center for GEFCC patients needing any number of laboratory services.

McHenry County Health Department: The Health Department will soon be leasing space in our McHenry Community Health Center, enabling this location to provide WIC and Family Case Management services to our patients.

Pioneer Center: Our McHenry Community Health Center professionals have collaborated with the Youth Service Bureau of Pioneer Center to help provide management and continuity of care for some of the children and adolescents that have been managed by the Bureau.

Presence St. Joseph: Presence has been collaborating with GEFCC since before we were even incorporated, helping drive community support to incorporate the budding organization and encourage its application to become a Federally Qualified Health Center. Presently, Presence St. Joseph has one of their professionals on our Board of Directors.

Quest Diagnostics: Quest Diagnostics leases space in our Streamwood Community Health Center, providing a comprehensive Patient Service Center for GEFCC patients needing any number of laboratory services.

Renz Addiction Counseling Center: As part of Renz’s Community Homeless Integration Project, GEFCC provides expanded access to health care for Renz clients of the Project.

Rosecrance: GEFCC, via our McHenry Community Health Center, and Rosecrance refer clients to each other, and collaborate regarding health education and service provision.

School District U-46 & District 300: GEFCC provides school-based primary health care and oral health care in a number of area schools each year.

Sherman Health: Sherman Health offers two members of their Executive Committee to serve on GEFCC’s Board of Directors, and GEFCC operates Health Centers in two buildings owned by Sherman (Our Slade Health Center at 901 Center Street and our Randall Health Center in Sherman’s Medical Office Building at 1435 N. Randall Road in Elgin). About 80% of GEFCC’s expectant mothers choose to deliver at Sherman Hospital.

St. Alexius Medical Center: About 20% of GEFCC's expectant mothers choose to deliver their newborn(s) at St. Alexius. In addition, a member of the Alexian Brothers Health System Executive Team is on our Board of Directors.

Thresholds: GEFCC, via our McHenry Community Health Center, and Thresholds extensively refer clients to each other, and collaborate regarding health education and service provision.

VNA Health Care: VNA jointly participates with GEFCC in the Kane County Family Case Management (FCM) Cluster, and the two organizations also work together regarding breast and cervical cancer screening services and referrals to GEFCC for oral healthcare services at our Dental Clinic.

Well Child Center: WCC sublets space from GEFCC’s Seneca Health Center for WIC (Women, Infant and Children) services offered to GEFCC patients.

YWCA of Elgin: GEFCC and YWCA Elgin extensively refer clients to each other, and collaborate regarding health education and service provision.
