GEFCC’s 2017 outcomes on the Bureau of Primary Health Care’s Clinical Indicators have again been recognized as among the best of all Federally Qualified Health Centers in Illinois and the United States – for the 5th year in a row!
The Department of Health and Human Services, Health Resources and Services Administration (HRSA) recognized GEFCC as a Health Center Quality Leader on August 22, 2018, with Greater Elgin Family Care Center being awarded its 5th consecutive Health Center Quality Leader award.
GEFCC also received the prestigious Advancing Health Information Technology for Quality award, in recognition of GEFCC’s comprehensive use of integrated HIT, both internally and through interoperable IT communication systems with external collaborating organizations. Also awarded to GEFCC was the Primary Care Medical Home Recognition and Electronic Health Record Reporter awards.
The Awards are based on GEFCC’s internal sophistication and 2017 clinical outcomes, which substantially outpaced the Illinois benchmark on 13 of 16 reportable Clinical Indicators. In addition, GEFCC’s clinical outcomes were also better than the National average of 13 of 16 Indicators.
GEFCC was also recognized by the Centers for Disease Control as a 2017 Million Hearts Hypertension Control Champion.
The recognitions again acknowledge that Greater Elgin Family Care Center’s operations and outcomes are superior to those of most Health Centers in the State and Nationally.
