Since its inception, Greater Elgin Family Care Center has collaborated extensively with other entities with the goal of increasing efficiency and minimizing duplication—all for the betterment of our patients. The following is a list of some of our current collaborations:
Activate Elgin: Activate Elgin is an initiative to engage all sectors of the community to work in harmony providing all residents with opportunities to improve their health, well-being, and overall quality of life. GEFCC proudly serves on Activate Elgin Coalition.
Advocate Sherman Hospital: Advocate Sherman Hospital is a long-standing partner of Greater Elgin Family Care Center. GEFCC operates our Randall Health Center in the medical office building adjacent to Advocate Sherman Hospital at 1435 N. Randall Road in Elgin). About 70% of GEFCC’s expectant mothers choose to deliver at Advocate Sherman Hospital.
Alexian Brothers Health System: Now AMITAHealth, Alexian Brothers and GEFCC enjoy a rich and substantial collaboration that includes a representative from AMITAHealth serving on GEFCC’s Board of Directors, and GEFCC leasing space from AMITA for our Lake Health Center in Hanover Park. In addition, GEFCC Obstetricians perform newborn deliveries and surgeries at both St. Alexius Medical Center and Alexian Brothers Medical Center.
American Cancer Society: GEFCC partnered with the American Cancer Society to bring educational workshops and materials to our patients on the topic of preventing colorectal cancer.
Aunt Martha’s Health & Wellness: GEFCC provides its quality, affordable primary health care services to homeless persons from Elgin with financial support from Aunt Martha’s.
Boys & Girls Club of Elgin: GEFCC offers both primary care and dental care to Boys & Girls Club members and parents through their annual health fairs.
Centegra Health System: Centegra leases space from GEFCC in our McHenry Community Health Center for their laboratory/patient service center. GEFCC Licensed Independent Practitioners have secure, private access to images and all health record information of McHenry Community Health Center patients who receive care, including ultrasounds, at Centegra Hospital McHenry.
Centro de Información: GEFCC provides health education for Centro clients at Centro’s offices twice monthly. Additionally, GEFCC offers both primary care and dental care to Centro clients through their annual health fairs.
Children’s Home + Aid: Greater Elgin Family Care Center and Children’s Home + Aid often refer clients/patients to each other for services.
Community Coordinated Child Care (4-C): Our McHenry Community Health Center professionals are actively working on a committee formed by 4-C to improve access to services available to those children of McHenry County in need.
Community Crisis Center: GEFCC provides primary care services to the residents of CCC in the shelter on a regularly-scheduled basis.
Community and Economic Development Association of Cook County, Inc. (CEDA): CEDA sublets space in GEFCC’s Creekside Health Center for WIC (Women, Infant and Children) services offered to GEFCC patients.
Cook County Health and Hospital System: CCHHS offers diagnostic and subspecialty services to GEFCC patients who reside in Cook County, including those that receive care at our Creekside Health Center and Streamwood Community Health Center.
DeKalb County Health Department: DeKalb County Health Department was instrumental in the planning and development of our Center for Family Health. GEFCC provides pediatric and family medicine care at our Center for Family Health – DeKalb, which is in the Health Department’s offices at 2550 N. Annie Glidden Road in DeKalb.
Dekamore Dental: Center for Family Health patients who need preventive and restorative oral health care are referred to familia Dental for services, in addition to our own Dental Clinic.
Ecker Center for Mental Health is where GEFCC refers many Elgin-area adult patients who are in need of outpatient behavioral health services. Conversely, Ecker Center refers their clients to GEFCC for primary health care, including adult oral health care, as needed.
familia Dental: Creekside Health Center patients who need preventive and restorative oral health care are referred to familia Dental for services, in addition to our own Dental Clinic.
Family Service Association of the Greater Elgin Area: In addition to GEFCC referring behavioral health patients to FSA for ongoing counseling and FSA referring clients who need primary health care to GEFCC, the 2 organizations enjoy a rich collaborative history regarding advocacy for under privileged persons.
Food for Greater Elgin: GEFCC regularly provides informational services at Food for Greater Elgin for area residents visiting their location, and conversely Food for Greater Elgin educates their guests about the quality and affordable primary health care services available at all GEFCC Health Centers.
Kane County Health Department: GEFCC serves alongside the Kane County Health Department to address the health needs of the residents of Kane County, through the Health Department’s Advisory Board, Healthy Places Coalition, All Our Kids (AOK) Network and the Kane County Breastfeeding Coalition, to name a few.
Kane County Family Case Management Cluster: GEFCC coordinates communication and integration amongst Kane County’s Family Case Management providers, including Aunt Martha’s Health & Wellness and VNA of Fox Valley.
Northern Illinois Food Bank: GEFCC partnered with the Northern Illinois Food Bank to host their Family Summer Mobiles in Elgin, IL and Sycamore, IL.
Northwestern Medicine KishHealth System: KishHealth was instrumental in the planning and development of our Center for Family Health. Northwestern Medicine Kishwaukee Hospital offers a wide-array of specialty and ancillary services to our Center for Family Health patients and the 2 organizations regularly refer patients to each other for services the other provides.
Laboratory Corporation of America: LabCorp leases space in our Health Centers, providing a comprehensive Patient Service Center for GEFCC patients needing any number of laboratory services via a bi-directional laboratory interface.
MEDPRO Mobile: MEDPRO provides mobile ultrasound staffing and services in several of our Health Centers.
McHenry County Health Department: The Health Department leases space in our McHenry Community Health Center, enabling this location to provide WIC and Family Case Management services to GEFCC patients.
Northwest Community Healthcare: NCH was instrumental in the planning and development of our Creekside Health Center in Wheeling. NCH offers a wide-array of diagnostic and subspecialty services to our Creekside Health Center patients, who also deliver their newborns at NCH. The 2 organizations regularly refer patients to each other for services the other provides.
Omni Youth Services: As a premier provider of mental health and substance abuse services for area residents, Creekside Health Center’s younger patients are referred to Omni for care, as needed.
Pioneer Center: Our McHenry Community Health Center professionals collaborate with the Youth Service Bureau of Pioneer Center to help provide management and continuity of care for some of the children and adolescents that are managed by the Bureau. In addition, Greater Elgin Family Care Center and Pioneer Center often refer clients/patients to each other for services.
Presence St. Joseph Hospital: Presence has been collaborating with GEFCC since before we were even incorporated, helping drive community support to incorporate the budding organization and encourage its application to become a Federally Qualified Health Center. Presently, Presence St. Joseph has one of their professionals on our Board of Directors.
Renz Addiction Counseling Center: As part of Renz’s Community Homeless Integration Project, GEFCC provides expanded access to health care for Renz clients of the Project. In addition, Greater Elgin Family Care Center and Renz Center often refer clients/patients to each other for services.
Rosecrance: GEFCC, via our McHenry Community Health Center, and Rosecrance refer clients to each other, and collaborate regarding health education and service provision.
Safe from the Start: GEFCC serves on Safe from the Start Coalition, and provides referrals to the Safe from the Start program.
School District U-46 & District 300: GEFCC provides school-based oral health care in a number of area schools each year. Representatives from GEFCC also serve on U-46’s Civic and Business Advisory Committee.
Thresholds: GEFCC, via our McHenry Community Health Center, and Thresholds extensively refer clients to each other, and collaborate regarding health education and service provision.
Two Rivers Head Start Agency: GEFCC offers both primary care and dental care to Two Rivers Head Start Agency students and parents through annual health fairs.
VNA Health Care: VNA jointly participates with GEFCC in the Kane County Family Case Management (FCM) Cluster, and the two organizations also work together regarding breast and cervical cancer screening services and referrals to GEFCC for oral health care services at our Dental Clinic.
Walgreens: Through their participation in the Federal Drug Pricing (340B) program, Walgreens offers Greater Elgin Family Care Center patients reduced-priced pharmaceuticals at all of their locations in GEFCC’s serice area.
Well Child Center: WCC sublets space from GEFCC’s Seneca Health Center for WIC (Women, Infant and Children) services offered to GEFCC patients.
YWCA of Elgin: GEFCC and YWCA Elgin extensively refer clients to each other, and collaborate regarding health education and service provision. YWCA is also a sub-grantee of GEFCC to assist in enrollment into the Affordable Care Act.
