Sliding Fee Discount

Greater Elgin Family Care Center, as a Community Health Center grantee, is required to offer a sliding fee discount program to all patients who are under 200% of the Federal Poverty guidelines based on reported income and family size.

All patients presenting who are below 100% of the Federal Poverty guidelines based on reported income and family size will receive a full discount. However, nominal fees may be collected from individual or families with an annual income at or below 100% of the Poverty Level.

To qualify for the sliding fee discount, patients must provide proof of identification, proof of family size and proof of income. Patients must renew this information every 12 months to continue to receive the discount.
