Primary Care Medical Home (PCMH)
You might hear staff talk about PCMH, maybe you’ve seen the word in your patient package, but you still wonder what does PCMH really mean and what does it mean for you?
Primary Care Medical Home or PCMH for short, is a designation that GEFCC worked very hard to achieve.
GEFCC became certified as a Primary Care Medical Home in 2007 by The Joint Commission, an independent, not-for-profit organization whose accreditations and certifications are recognized nationally as a symbol of quality and high medical standards.
To be a PCMH means that our patients are at the center of what we do.
It means we are committed to providing:
